Motivational interviewing efficacy on strengthening ischemic heart disease patients’ motivation to change unhealthy behavior during rehabilitation period
The efficacy of psychological intervention known as motivational interviewing for strengthening ischemic heart disease patients’ motivation to change unhealthy behavior during rehabilitation period is analyzed in this doctoral dissertation. The peculiarities of ischemic heart disease problems and possibilities of its secondary prevention during rehabilitation are discussed in theoretical part of the dissertation. Also, the sociodemographic, somatic, health related and psychological risk factors related to this disease are analyzed. One of the most suitable psychological interventions used to change unhealthy behavior in rehabilitation period motivational interviewing is presented. The efficacy of this method and its relation to psychological, somatic and sociodemographical factors is analyzed in literature review. The empirical part of this dissertation presents the results of quasi-experiment which was held in Cardiac department of Abromiškės rehabilitation hospital. Systematic longitudinal study was employed at three points in time: at the beginning and at the end of rehabilitation and three months after it. 362 patients agreed to participate and completed all baseline measures and this is the main study’s sample. It consisted of 191 patients in experimental and 171 in comparative group. The results of this research disclosed that motivational interviewing in an efficient method which strengthens ischemic heart disease patients’ motivation to implement physical activity and diet changes during rehabilitation treatment period. Moreover, motivational interviewing’s efficacy is related to such personality traits as neutoticism and agreeablesness, and with emotional state.
Daktaro disertacijoje analizuojamas motyvavimo pokalbio, kaip psichologinio intervencijos metodo, veiksmingumas, stiprinant sergančiųjų išemine širdies liga motyvaciją keisti sveikatai nepalankų elgesį reabilitacijos laikotarpiu. Disertacijos teorinėje dalyje pristatomas išeminės širdies ligos problematikos aktualumas bei nagrinėjamos antrinės išeminės širdies ligos prevencijos galimybės reabilitacijos metu. Taip pat pristatomi sociodemografiniai, somatiniai, su elgesiu susiję bei psichologiniai šios ligos rizikos veiksniai. Darbe, kaip viena tinkamiausių intervencijų reabilitacijoje, siekiant koreguoti sergančiųjų išemine širdies liga sveikatai nepalankų elgesį, pristatomas motyvavimo pokalbis. Kitų mokslinių tyrimų kontekste analizuojamas šio metodo veiksmingumas ir sąsajos su psichologiniais, somatiniais bei sociodemografiniais veiksniais. Empirinėje darbo dalyje pristatomi 2013 m. – 2016 m. VšĮ Abromiškių reabilitacijos ligoninėje atlikto kvazieksperimento rezultatai. Tyrimą, kuris atliktas pakartotinių matavimų sistemingosios atrankos būdu, sudarė trys etapai: reabilitacijos pradžioje, pabaigoje ir praėjus trims mėnesiams po reabilitacijos. Pagrindinę tyrimo imtį sudarė 362 tiriamieji, iš kurių 191 priskirtas poveikio grupei, o 171 – lyginamajai. Tyrimo rezultatai atskleidė, jog motyvavimo pokalbis yra veiksmingas metodas siekiant stiprinti išemine širdies liga sergančiųjų motyvaciją vykdyti fizinio aktyvumo ir mitybos pokyčius reabilitacijos laikotarpiu. Taip pat nustatyta, jog motyvavimo pokalbio veiksmingumas siejasi su asmenybės bruožais neurotiškumu ir sutarumu bei su emocine būsena.
